What information do we collect and how?
Here at Farndon Physiotherapy Ltd, we take your privacy seriously.

If you leave demographic information, such as name, contact telephone number, email address or home address on either our answer phone service, email, WhatsApp, Messenger, or SMS text, either our administrative staff or clinical staff will use this information to contact you to provide information about the service or to offer an appointment. Once an appointment is booked, this information will be entered into our Cliniko practice management software, to allow booking and management of appointments and to contact you regarding services which Farndon and Tattenhall Physiotherapy and Wellbeing can offer related to your enquiry.

If you ring and request a service provided by another therapist or business working within Tattenhall Physiotherapy and Wellbeing or Farndon Physiotherapy Clinics, we will pass the contact details you provided to the relevant service provider, for them to contact you directly. These services have their own Privacy Notice, which you can discuss with them. Once your details have been passed on, they will be deleted from our system and only be saved if you have made a Physiotherapy appointment.

On your Physiotherapy assessment, personal information is gathered using Cliniko practice management software. Any information related to your general health and the relevant to the problem you have attended, information regarding any diagnoses, ongoing treatment, and progression can only be viewed by therapist/clinician who is currently treating you and is kept private. Farndon Physiotherapy Ltd. is a customer of Cliniko and remains as the Data Controller. Cliniko is classed a Host and Data Processor and do not access your medical information. How is your information used?
Your information enables us to provide you with our services. Examples of how we use your information responsibly include:

Contact details such as address, phone numbers and/or email address, are collected to enable appointments booking, changes of appointment or pre-arranged communication regarding your health/condition.

To identify your record reliably and accurately in the future for follow up treatment.

​Personal sensitive data relating to your health is gathered as part of your physiotherapy assessment, to aid in diagnosis and treatment of your condition and can only be seen by your treating clinician/therapist.

Communication with GP’s, Consultants, healthcare professionals, your medical insurance company, and legal services – with your consent (see access to information).

On booking your appointment, we will ask you if you are happy to receive any information emails in relation to the services we have to offer in our clinics. If you are not happy to receive these emails, please let us know when booking your appointment, or when you attend for an assessment.

Internal training and audit purposes – in which event, information will be anonymised.
Who has access to your information?
Any demographic information, such as name and contact details can only be used by Farndon and Tattenhall Physiotherapy and Wellbeing staff and or services it provides via other businesses.
Your clinical/sensitive information will not be shared outside Farndon Physiotherapy Ltd, incorporating Tattenhall Physiotherapy and Wellbeing unless your consent is obtained.

We will not sell or rent your information to third parties, nor will we share your information with third parties for marketing purposes without your consent.

How is your information kept?
When you give us personal information, we take steps to ensure that it’s treated securely. Clinical/health information is kept securely for 8 years after the date of your last treatment or at age 25 years for patients under 18 years of age. After this date it will be securely disposed of.
How can you access and update your information?
You have the right to see the information we have relating to you. You also have the right to amend any inaccuracies in the information we hold. You have the right to ask for a copy of the information Farndon Physiotherapy Ltd. hold about you.
